Your completed Intake Form will help us to establish a solid foundation in understanding your NDIS Diagnosis, Specific Support + Care Needs & your Program Preferences.
From here we will finalise:

As a first step we will gather some information from you with our Vision + Goal Questionnaire to help us gain a deeper understanding of your unique goals, interests, aspirations, priorities & desired outcomes.
In collaboration with you, we clarify, flesh out + finalise the details of your program & booking.
This assists us to tailor your program ensuring a holistic and enriching experience that aligns with your unique personal growth direction.
We carefully appoint a support worker who is suitably qualified and resonant with your values, needs, and what you’re hoping to cultivate through this work.

All of our programs are available through Core Funding, delivered within a 1:1 support framework.
